Bit of a long shot but here goes, I'm after some cam followers for a vintage Italian engine. The followers are 22mm diameter by about 40mm long, I could grind followers that are slight bigger or maybe sleeve the bores....has any one had follers hard welded & reground? Any suggestions?

I'm assuming you need non-hydraulic cam followers? If you're going the sleeve route and your block has enough meat for a slightly bigger diameter, Ford Cologne V4/V6 tappets are 22.2mm x 47mm; they're solid and have plenty material that can be ground off I suppose (face may need re-hardening though?). New ones are not that expensive, and I have cheap used ones if required.

I think later Ford crossflow (711M) might be 22.2mm as well. I'd have to dig one out tonight to check.

Could you get a piece of solid bar cut down by a machine shop (or by a fricking laser)?

Off the shelf bar stock won't cut it I think, solid tappets are ground castings I suppose?

Don't grind the followers, just hone the lifter bores out to suit a 22.2mm lifter if a suitable length one can be found. If you grind anything off the lifters you'll end up going through the hardening and they'll self destruct very quickly and most likely damage the rest of the engine at the same time.
You can get pushrods made relatively easily to get the geometry back in range.

Depending on your cam/tappet design, you may or may not need to think about the 22 foot (or whatever other ludicrous number) radius on the cam-facing bottom of the tappet.. The Tobin-Arp / Sunnen TR-6700 was invented to sort this..
On the other hand this post might just be an opportunity to share a favourite workshop machine video....
