Sorted 
Took the filled out V5-2 form to a local post office, and was told that if I want to fill out a V62 form, I can't tax it at the same time, as the original V5 is lost, and I can only tax it when a new V5 has been issued in my name.
Went to a different post office, didn't mention the V62, and managed to tax it on the V5-2. Will sort out applying for a new V5 with a V62 form tomorrow
