Its hard to say without seeing it in the metal but it does look to me like the metal has creased and stretched.
I think that could be quite hard to pull out, One of them suction cup pullers wouldn't be able to get enough purchase. A slide hammer would do it but you'd have to drill several holes to use it unless you can get one of the ones that welds itself to the panel.

That would be a job for a pin puller. Basically you bare metal along the length of the dented area, and with a special tool you weld pins along the deep section of the dent, and use a slide hammer to slowly ease the dent out. You then cut or twist the pins off, then skim it over with filler. It's more of a specialist job unless you have a pin puller. It definitely won't pop out, even if you could get behind it, it's too sharp. If you're not being fussy with the van, you could always fill it.. just bare metal it first or the filler could lift and/or crack.

we did it like this: Tool in the last picture is a 'spotter'. a mix of a spot elder and sliding hammer. Welds itself to the metal, you pull, twist it to brake loose and repeat - untill the dent is out. A verry, verry usefull tool to posess - and as soon as I can afford one I will buy one If you fill it, lead load it. I would not use filler... But that'spersonal prefreence; I like to fix metal with metal and not with plastic. Either way it's a paint off-dent out-paint on operation I guess. Before: After: Needed only a slight skim of lead to finish it before painting

Heating it won't help, you're more likely to make it worse by warping it. Filling it isn't ideal, but if you're not being fussy it can be fine, do it properly and it won't fall out or crack like old filler used to. Lead loading something like that on an old van is just silly.

Why not take the sink/bunks out i know its hassle but it will save you a lot of trouble then you can knock most of it out and just skim over with a bit of filler

No that is a lot of hassle cause of the way i built the camper all the 12v 240v and water is on that side also the floor will have to come up and a lot of insulation to also i think the dent is just below the floor line of the van
