Did this on a cambelt cover bolt which was behind the crankshaft pulley. It was only when the spanner hit the pulley did I realise there was no way of going the other way as it was a flippy style ratchet spanner!! I had to beat it off with a hammer - ruined it, and bent the bolt.

A good few years ago, Halfords did twin-headed ratchet spanners. A total of 4 sizes per ratchet, with two ratchets in the set = 2 ratchets, 8 sizes!! Originally a rather silly £40-ish, but they went obsolete, so me and the manager got a set each for a FIVER! Good quality pieces of kit, undone many a tough bolt! i have a set of those, and they are great for going to scrapyards as you don't have to carry loads of tools with you, just these 2 spanners (plus a couple of screwdrivers for trim etc)

These are the 4 sizes per spanner ones... £40 but so worth it just for a little portable tool kit or keeping in the glove box Like pandaselecta said, great for scrappies. In fact I bought mine after going to a scrap yard and the lad who was working there had a pair and I was sold, got about 8 things out of a ford mondeo engine bay just using on of them...
