I’m intrigued into purpose of these as they might address my problem of a cable that’s too late long since the axle was brought forward for lowering.
Do you simply twist it around brake cable like a key to a can of spam?

yes if you look at the item you put the cacle in a triangle type form , then take up the slack on the cable by turning the screw ,

As the picture shows in from the left under the hook then over the cup and under the other hook then you move the wheel to push the cup up to take the slack out of the cable.
If you have a boat load of slack you could fit two of them or three if you really have to.

I can't work out how the cable stays in the upper hooks as it would pull on the tightest path?

screw the screw back cable goes across the 3 lips , then take up slack by adjusting the screw out to where you want to take up slack in cable 11 left or deal on the lot ,

Blimey I remember seeing an old Cortina or Escort with three of those things hanging from its knackered handbrake cable - I also thought they were made an instant MOT fail. Good old bit of retro bodging nostalgia.
